Outreach is there for anyone over 50 years old who is living with dementia, is struggling with mobility or has poor health, as well as those who care for them, to give support at home or out and about.
We can provide support to clients living in their own home or in a care home.
We can visit you at home to help you keep up with much loved hobby or activity, have a cuppa and a chat, or support you to do things around your home.
As registered care workers we can also assist you with getting up and ready for the day and having a bath or shower.
We can support you to get out and about and do the things you enjoy, be that going out for a walk and a coffee or visiting a museum or art gallery. Whatever you fancy doing- we will try and make it happen!
We can also help you to do the things you need to do- like attend appointments or run errands.
As registered care workers we can help with any care and support needs you may have and if needed we can organise a taxi or support you to use the bus.
As a carer, it’s important to get a well deserved break. We can spend time with your loved one at your home or we can go out and about, leaving you to relax in the knowledge that they are in safe hands.
Our care workers can help your loved one with any care and support needs including eating a meal, using the toilet, having a bath or shower and taking prescription medications.
